Health Information Management Department

The Health Information Management Department is responsible for maintaining medical records in a standardized and professional manner in order to protect patient confidentiality while allowing adequate access to providers in order to promote quality patient care.

Release of information, transcription, diagnosis coding and electronic medical record scanning are just some of the major duties performed in the Health Information Management Department.

The Health Information Management Department is responsible for the processing, handling and maintaining of all hospital medical records. Our department is open from 8:00 a.m. to 4:00 p.m. Monday through Friday.

Our program

The Health Information Management Department plays a very important role in the health care industry. The following are a list of some of the pivotal services and functions that the HIM Department provides at Bradford Regional Medical Center:

Release of Information : 814-362-8253

Medical records are released in accordance with State and Federal laws. To receive copies of you or your children’s records, you must first sign an Authorization for Release of Information form granting us permission to release the said records. Our office will provide you with the appropriate HIPPA compliant Authorization needed to complete this request. We ask that, when picking up copies of you and/or your children’s medical records, you provide photo identification. If you have Power of Attorney for an individual and are requesting copies of his/her records, please provide a copy of the same.

You have a right to request copies of records for your dependent children if they have not yet reached the age of 18. Once 18-years-old, a person must sign an Authorization for their own records.

Certified birth certificates are NOT available from Bradford Regional Medical Center’s Health Information Management Department. These records must be ordered from the Department of Vital Statistics in the State which the birth was registered.

• Coding of Medical Records (814) 368-7767
  The Health Information Management currently has four (4) staffed coding professionals who assign diagnosis codes for all patient account types. Due to the latest technology in healthcare, we have remote coding capabilities.

• Data Analyzing (814) 368-2301
  Rhawnie Barrett is the data analyst for the Health Information Management Department. Employees and medical staff who have inquiries regarding completion and/or deliquencies of medical records can contact Rhawnie for further information.

• Scanning (814) 368-2015 and (814) 368-2017
  The Health Information Management Department currently staffs two (2) scanners. These individuals are responsible for scanning the paper medical record into the computer and converting the same into an electronic medical record. These individuals do a very thorough quality check to ensure 100% accuracy of all health records.
